안녕하세요! 이번에는 벨기에 비자 제출서류 중 하나인 비자 신청서 작성에 대해 알아보도록 하겠습니다.

비자 신청서가 영어로 적혀있지만, 작성해야하는 내용이 많아 어떻게 해야할 지 막막하게 느껴질텐데요,

 

비자 신청서 양식에 어떤 내용을 작성해야하는 지 지금부터 알아보겠습니다!

1. Surname(Family Name) : 성 ex) HONG

2. Surname at Birth : 공란으로 비워두셔도 됩니다.

3. First name : 이름 ex) GIL DONG

4. Date Of Birth : 생년월일 (일/월/연도 순) ex) 30/08/1992

5. Place of Birth : 태어난 도시

6. Country of Birth : 태어난 국가

7. Current nationality (nationality at birth, if different) : 현재 국적 (만약 다를 경우 태어난 국가를 적어주세요)

8. Sex : 성별 (남자/여자)

9. Marital Status : 결혼여부를 체크해주세요

  □ Single : 미혼

  □ Married : 기혼 

  □ Separated : 별거 중

  □ Divorced : 이혼

  □ Widow(er) : 사별 

  □ Other (please specify) : 기타

10. In the case of minors: Surname, first name, address (if different from applicant's) and nationality of parental authority/legal guardian : 공란으로 비워두셔도 됩니다. 

11. National identity number, where applicable : 공란으로 비워두셔도 됩니다. 

12. Type of travel document : 여권의 종류 -> Ordinary passport에 체크!
